2) I write up my own notes on the conversation and e-mail it to THEM,
saying "Here's what I remember of our conversation. If I got anything
wrong please let me know; otherwise this is what I'll be doing."

The only thing I might suggest to that is when you email anyone under CC not BCC make a copy to yourself. That way the client sees your keeping records of the conversation. I've also had situations where the email wasn't received and all I had to do was forward my copy. I keep these a special folders natually
